What behavioral health and substance use services does Stacked Assessments offer?

Stacked Assessments provides a full range of behavioral health assessments, substance abuse evaluations, DOT SAP (Substance Abuse Professional) evaluations, Oklahoma DUI/ADSAC assessments, and a 24-hour DUI education class.

Do I need an appointment for a DOT SAP evaluation or ADSAC assessment?

Yes, for DOT SAP evaluations and Oklahoma ADSAC assessments, an appointment is required. This ensures your substance use evaluation is conducted thoroughly. For the 24-hour DUI class, you can attend without an appointment — just arrive 10–15 minutes early on your first day.

Can I use a payment plan for DUI class or substance use assessment?

Absolutely. We offer flexible payment plans for many of our services. For our 24-hour DUI program, you only need $60 to start, and you can pay the remaining balance as you complete your sessions.

How do I schedule a DUI assessment, ADSAC evaluation or SAP evaluation?

You can book online using our secure scheduling system, or call/text our office directly. We provide both in-person assessments and telehealth substance use evaluations for convenience and confidentiality.

Are telehealth options available for substance use and behavioral health evaluations?

Yes — Stacked Assessments offers secure telehealth evaluations for DOT SAP assessments, behavioral health screenings, follow-up substance use assessments, and court-ordered substance abuse evaluations. Telehealth makes compliance faster and more accessible.

What documentation is required for my assessment (SAP, ADSAC, or DUI)?

Having these items ready helps you complete your assessment smoothly and efficiently.

Please bring:

  • Valid photo ID

  • Court or probation documents (if applicable)

  • Driving record (especially for ADSAC assessment)

  • Any relevant DOT or employer forms

How long does a behavioral health or substance use assessment take?

Most assessments — whether it’s a SAP evaluation, ADSAC evaluation, or a general mental health screening — typically take 45 to 75 minutes, depending on documentation and the nature of the evaluation.

What payment methods do you accept for evaluations and DUI classes?

We accept major credit/debit cards as well as digital payment methods. When enrolling in the DUI class or other services, we can also set up payment plans as needed.

Can you explain the DOT SAP evaluation process?

We guide clients through every step for full federal compliance. The DOT SAP evaluation process includes:

  1. Initial substance use assessment

  2. Required treatment or education

  3. Follow-up evaluation

  4. Return-to-duty recommendation

  5. A follow-up testing plan to ensure ongoing DOT compliance

Do you report assessment results to courts, employers, or the state?

Yes. We submit necessary reports for DOT SAP compliance, ADSAC DUI assessments, and court-ordered substance abuse evaluations, in accordance with legal and confidentiality requirements.

Can I start the 24-Hour DUI class without an appointment?

No appointment is needed, but book online for DUI classes to get a short registration form. You can attend up to two classes per week, but not two on the same day. This makes scheduling easier.

How long do I have to complete the 24-Hour DUI class?

You have 6 months from your start date to complete the class, per Oklahoma state rule. Be sure to finish all hours within that period, or you may forfeit both your time and payment.

Is my evaluation and assessment information confidential?

Yes. All our substance abuse evaluations, behavioral health assessments, and DUI education records are handled in strict accordance with HIPAA and professional confidentiality standards. We only release information when legally required or with your explicit consent.
